ESignDocumentTemplateUploadRequest

POST a new ESign Document Template into DocStorage for a given Practice or Clinic. If a document of that type already exists for that practice or clinic, a new version will be created.

The following routes are available for this service:
POST, OPTIONS	/ESignAPI/ESignDocumentTemplate/{DocumentTypeName}/{Practice}/{ClinicPk}
POST, OPTIONS	/ESignAPI/ESignDocumentTemplate/{DocumentTypeName}/{Practice}

To override the Content-type in your clients HTTP Accept Header, append ?format=json

To embed the response in a jsonp callback, append ?callback=myCallback
